IMMIGRATION MEDICAL EXAM

​

Our clinic is approved Civil Surgent center for US Citizenship and Immigration services (USCIS). Conducted by an approved physician designated by USCIS (also known as a "civil surgeon").

​

Our services include physical exam, immunizations, blood work up and filling up I-693 form. Same day appointments available and fast turn around times.

​

Please bring all necessary paperwork, including:

  •    Passport or I.D. card, travel documents, etc

  •    All personal medical history  

  •    Vaccination history, Blood work, X-rays, Ultrasounds, etc

  •    Form I-693

  •    Any other pertaining documentation

Immigration Medical Examination Fees:

​

Our fee is $199.00 for physical examination and filling out a I-693 form.

​

(all other necessary exams, blood work, immunizations are separate fees based on individual necessities)
