IMMIGRATION MEDICAL EXAMINATION
Our clinic is approved Civil Surgeon center for US Citizenship and Immigration services (USCIS). Conducted by an approved physician designated by USCIS (also known as a "approved civil surgeon").
Our services include physical exam, immunizations, blood work up and filling up I-693 form. Same day appointments available and fast turnaround times. The Immigration Medical Examination entails a review of your medical history and a physical examination. The doctor will complete a comprehensive examination that will include a review of your medical history and a physical examination.
Please bring all necessary paperwork, including:

Passport or I.D. card, travel documents, etc

All personal medical history

Vaccination history, Blood work, X-rays, Ultrasounds, etc

Form I-693
-
Any other pertaining documentation
After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. You will get a copy of the completed Form I-693 for your personal records before the doctor seals the envelope.
Immigration Medical Examination Fees:
Our fee is $199.00 for physical examination, Tuberculosis Test, ALL blood work up and completing a I-693 form.
(all other necessary exams, blood work, immunizations are separate fees based on individual necessities)
