Our clinic is approved Civil Surgeon center for US Citizenship and Immigration services (USCIS). Conducted by an approved physician designated by USCIS (also known as a "approved civil surgeon").

Our services include physical exam, vaccination records review, blood work up and completing I-693 form. Same day appointments available and fast turnaround times. The Immigration Medical Examination entails a review of your medical history and a physical examination. Doctor will complete a comprehensive examination that will include a review of your medical history, immunizations and a physical examination.

Please bring all necessary paperwork, including:

•    Passport or I.D. card, travel documents, etc

•    All personal medical history  

•    Vaccination history, Blood work, X-rays, Ultrasounds, etc

•    Form I-693

•    Any other pertaining documentation

After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. You will get a copy of the completed Form I-693 for your personal records before the doctor seals the envelope.

Do you accept insurance?
The basic $199 package is self-pay. Some additional vaccinations services may be eligible for insurance, depending on your insurance.

How long does the exam take?
Typically 45–60 minutes, depending on your history and the tests needed.

​

How long is the Form I-693 valid?
Typically valid for 2 years from the date the civil surgeon signs, unless USCIS specifies otherwise. (Check USCIS guidance.)

Can I walk in or must I book an appointment?
We strongly recommend booking ahead to ensure all tests can be completed. Same-day appointments are often available, but not guaranteed.
